In France in 2021, 90% of pregnant women chose to undergo screening for Trisomy 21, and 128,958 women benefited from a fetal aneuploidy screening test based on the analysis of cell-free DNA (cfDNA) in maternal blood. At the beginning of its use, this analysis was limited to screening for Trisomy 21, but it now allows the study of all chromosomes (expanded screening). More than half of fetal chromosomal abnormality screenings are expanded tests, and this practice continues to grow. In oncology, circulating tumor DNA (ctDNA) is studied for the detection, prognostic evaluation, and monitoring of the effectiveness of certain treatments. The high-throughput sequencing tools used for aneuploidy screening during pregnancy are likely to detect malignant diseases. Cancer is associated with pregnancy in 1 in 1,000 to 1 in 1,500 pregnant women, and the spread of expanded aneuploidy screening during pregnancy makes it possible to detect maternal cancers, including at infraclinical stages. This study will therefore help manage situations involving difficult-to-interpret results, such as suspected maternal cancer. It will make it possible to identify specific chromosomal abnormalities to be tested, which could potentially be included in future recommendations. In a second stage, it could contribute to harmonizing the practices of laboratory specialists performing fetal chromosomal abnormality screening using cfDNA.

Quadriceps Muscle Quality and Clinical Outcomes in Knee Osteoarthritis

The purpose of this cross-sectional, observational study is to evaluate the relationship between quadriceps muscle quality-specifically muscle echogenicity and thickness change during isometric contraction-and clinical outcomes such as pain, physical function, quality of life, and Patient Global Assessment (PtGA) in female patients with knee osteoarthritis. Participants will undergo a non-invasive ultrasound evaluation of their thigh muscles and complete standard clinical questionnaires to help determine how muscle structural and functional properties impact the daily lives of patients with knee osteoarthritis.

Pregnancy Birth Fear

Birth fear refers to intense feelings of anxiety, fear, and worry regarding the childbirth process, often arising from concerns about physical pain, complications, or uncertainty about the baby's health. Many women experience this fear during pregnancy, which, in some cases, may develop into a clinical condition known as tokophobia . Birth fear can increase stress levels, disrupting the mother's hormonal balance, which may prolong labor, increase the risk of complications, and negatively impact fetal development. Additionally, it can lead to issues such as loss of self-confidence, anxiety, and depression in the mother, while causing communication problems and lack of support between partners. Birth fear affects not only the labor process but also the postpartum period, potentially leading to postpartum depression, trauma, and difficulties in mother-infant bonding. All these factors can adversely impact the psychosocial well-being of both women and couples. Therefore, it is crucial to develop interventions aimed at recognizing and reducing birth fear.

Can External Vagus Nerve Stimulation Reduce Systemic Levels of Inflammatory Mediators in Duchenne Muscular Dystrophy Patients?
Duchenne Muscular Dystrophy (DMD)

The intended investigation is a pilot study to evaluate the safety and efficacy of a novel transcutaneous auricular vagus nerve stimulator system, termed TRAVAGUS ONE, to reduce systemic levels of inflammatory mediators in patients with Duchenne muscular dystrophy (DMD). Electrical vagus nerve stimulation is an investigational anti-inflammatory therapy targeting the nervous system to modulate dysregulated inflammation. DMD is a severe genetic disorder characterized by progressive muscle degeneration and weakness due to the alterations of a protein named dystrophin that helps keep muscle cells intact. The disease affects male children, and the symptom onset is in early childhood. In addition to the muscle degeneration all patients suffer from severe systemic inflammation and express increased systemic levels of proinflammatory molecules, which can be quantified in peripheral blood samples. Daily, systemic corticosteroid therapy with high doses is the standard of care in DMD to control symptoms and to slow disease progression through potent anti-inflammatory activity. Unfortunately, high dosage and long-term use of corticosteroids are typically also accompanied by severe adverse effects that reduce the quality of life in DMD patients. There is thus a great need for improved anti-inflammatory treatment with less severe adverse effects. In the planned pilot study involving 20 DMD patients aged 5-17 years, the investigators intend to treat each patient for one week in their home environment using transcutaneous auricular vagus nerve stimulation (taVNS) with a novel device named Travagus One to find out whether this intervention is safe and may reduce systemic levels of proinflammatory molecules. Venous blood samples will be collected at three different time points before and after the taVNS treatment period. Note: This study relates to an FDA-nonregulated Device. There are no U.S. Locations for the study. The study was approved by the Swedish Medical Products Agency.

Isotretinoin vs hCG for Male Infertility Due to Low or Absent Sperm
Oligospermia Azoospermia, Nonobstructive Infertility

This study compares two medical treatments to see how well they improve sperm production in men with infertility due to low sperm counts (oligospermia) or no sperm in the semen due to non-obstructive causes (nonobstructive azoospermia). Some men with these conditions have limited nonsurgical treatment options, and current therapies do not work for everyone. Participants in this study will be adult men ages 19 to 50 who have been diagnosed with oligospermia or nonobstructive azoospermia and do not have a correctable cause for infertility. Eligible participants will be randomly assigned to receive one of two treatments for three months: oral isotretinoin taken twice daily, or human chorionic gonadotropin (hCG) injections given three times per week. hCG is a standard hormonal therapy used in certain types of male infertility, while isotretinoin is being studied for its potential role in stimulating sperm production. At the start of the study, participants will have blood tests to measure reproductive hormone levels and a semen analysis. Blood tests will be repeated at one month and three months, and a repeat semen analysis will be performed at three months, which corresponds to a full cycle of sperm development. The main goal of the study is to determine whether motile (moving) sperm appear in the semen after treatment. Additional goals include measuring changes in sperm count and hormone levels, as well as monitoring medication side effects. Both study medications are FDA-approved and have known side effect profiles. Participants may or may not experience improvement in sperm production. The results of this study may help guide future treatment options for men with infertility due to low or absent sperm production.

Effects of Glycemic Control and Clinical Parameters on Tibial Nerve Conduction Latency in Diabetic Peripheral Neuropathy

This observational cross-sectional study aims to investigate the effects of glycemic control and clinical parameters on tibial nerve conduction latency in individuals with diabetic peripheral neuropathy. Adults aged 45 to 76 years with diabetic peripheral neuropathy will be evaluated at a neurology and clinical neurophysiology clinic. Data collection will include demographic and clinical characteristics, diabetes type and duration, fasting blood glucose, and HbA1c values obtained from routine clinical records. Participants will also be assessed using the Michigan Neuropathy Screening Instrument (MNSI), including both the questionnaire and physical examination components. Nerve conduction studies will be performed using standard electromyography procedures, and tibial nerve conduction latency will be recorded. The study will examine the relationship between tibial nerve conduction latency and glycemic control parameters, neuropathy screening scores, and diabetes duration. The findings may help improve early identification and clinical evaluation of diabetic peripheral neuropathy.

Facilitation of Information eXchange for Shared Decision Making for Lung Cancer Screening
Lung Cancer Screening

The goal of this pilot clinical trial is to learn whether the patient and provider support program, called FIX-SDM, helps patients and providers engage in shared decision-making for lung cancer screening during primary care visits and increases the number of patients who complete lung cancer screening. The investigators will also assess the acceptability of the support program and the feasibility of the study protocol to prepare for a future large-scale trial. The main questions this trial aims to answer are: * Does the patient and provider support increase the number of patients who complete lung cancer screening? * Does the patient and provider support help patients and health care providers engage more in shared decision-making and improve the quality of the patient's decision regarding lung cancer screening? * Is the study protocol feasible? The investigators will compare the patient and provider support program to usual care to see if the support increases the number of patients who complete lung cancer screening. Primary care provider participants will: * Receive the provider support session and educational materials, or follow usual practice * Answer a baseline survey and a follow-up survey in 6 months * Answer additional survey questions regarding the acceptability of the provider support session if they receive it Patient participants will * Receive a smoking history survey, a decision aid, and text messages about lung cancer screening prior to the primary care visit, or receive usual care * Complete the baseline survey and two follow-up surveys, one right after the primary care visit and another 3 months after the visit.

Evaluation of Serum Autophagic Biomarkers in the Acute Response to Walking and Cycling in Healthy Male Individuals
Effects of Walking and Cycling on Serum Physiological Biomarkers

Physical exercise (walking and cycling) is a potent physiological stimulus that simultaneously alters energy balance, mechanical loading, and metabolic demands in the organism. Autophagy is recognized as a fundamental mechanism in the regulation of acute cellular responses to such stimuli, playing a critical role in maintaining cellular homeostasis, removing damaged proteins and organelles, and ensuring metabolic adaptation \[1\]. Experimental and translational studies have demonstrated that particularly moderate-intensity and controlled mechanical loading can activate autophagic pathways, thereby supporting structural and functional adaptation in muscle, bone, and connective tissues \[2-4\]. In the current literature, the relationship between autophagy and exercise has largely been evaluated through experimental animal models and a limited number of human studies \[5\]. Although animal studies have clearly shown that physiological loading such as walking and running increases molecular signals associated with autophagy, the direct assessment of autophagy at the tissue level in humans is limited due to ethical and feasibility concerns, as it requires invasive methods (e.g., muscle biopsy) \[3\]. Therefore, recent human studies have increasingly focused on indirect evaluation of autophagy through peripheral blood mononuclear cells and circulating biomarkers \[6\]. Indeed, recent human studies have reported that proteins associated with autophagy may exhibit changes in peripheral blood cells or circulation in response to acute exercise, and that this response may vary depending on the type, intensity, and mechanical characteristics of the exercise \[6\]. These findings suggest that autophagy is not merely a tissue-specific process but can also be monitored at the systemic level as part of physiological adaptation \[7\]. However, there is a limited number of human studies that comparatively investigate the acute effects of different exercise modalities with distinct mechanical loading profiles (such as walking and cycling) on serum biomarkers related to autophagy. In this context, the present project aims to evaluate the acute effects of two common aerobic exercise modalities with different mechanical loading characteristics-walking and cycling-on serum biomarkers associated with autophagy in healthy male individuals. In this study, Beclin-1, LC3, and ATG3 levels will be considered not as direct indicators of autophagic flux in tissues, but as circulating biomarkers associated with the initiation and maintenance of autophagy. By examining changes in the serum levels of these proteins following acute exercise, it is aimed to obtain indirect yet biologically meaningful data regarding exercise-induced cellular adaptation mechanisms without the need for invasive procedures. In conclusion, this study aims to contribute to an important gap in the current literature by safely and ethically demonstrating the acute physiological responses related to autophagy in humans across exercise modalities with different mechanical characteristics, thereby enhancing our understanding of the exercise-autophagy relationship.

Mesenteric Vascular Preconditioning Combined With Laparoscopic and Endoscopic Cooperative Surgery for Submucosal Tumor at the Gastroesophageal Junction

This study is a single-center, prospective, single-arm, exploratory clinical study (phase II exploratory trial). No parallel control group was established in the study, and all subjects who met the inclusion criteria underwent a uniform surgical treatment approach, namely, double-scope combined resection with mesenteric vascular preconditioning. The study aims to preliminarily evaluate the safety and effectiveness of this novel surgical strategy for the treatment of submucosal tumor at the gastroesophageal junction, and to provide data support for subsequent larger-scale controlled studies.

Thermal Measurements of the Gut With a Colonoscope Compatible Thermal Probe

The goal of this clinical trial is to use a modified thermal probe to measure temperature rise in the colonic mucosa of participants with inflammatory bowel disease, Crohn's disease, and/or ulcerative colitis. The main question it aims to answer is: Is the thermal probe an effective device to use to detect temperature rise in the colonic mucosa? During the participant's standard of care colonoscopy, the thermal probe will be inserted into the colonoscope. The thermal probe is connected to a temperature transmitter that collects and saves the temperature of the colon in real time.

Insulin Intratympanic Application
Tympanic Membrane Perforation

The objective of the present study was to study the impact of insulin insitu-gel formulation on healing progress after surgical procedures for treatment of dry tympanic membrane perforation for at least three consecutive months. At the beginning, an insulin insitu-gel was prepared from three different polymers compatible with insulin physiochemically and reported to be vain on insulin efficacy or safety. Insulin insitu-gel was prepared using chitosan, polyvinyl alcohol, and Pluronic F127 with glycerol as a penetration enhancer. Selection of an optimized formulation through in situ gel characterization and in vitro drug release kinetics then the formulation was evaluated for healing efficacy on human volunteers in RCT. A randomized controlled clinical study was implemented for clinical evaluation of insitu-gel formulation activity in the Department of Otorhinolaryngology, Minia University Hospital, from January 2025 to March 2026. Perforation healing signs as a primary indicator was followed and reported in three follow-up visits (1 week, 1 month, and 2 months) in addition hearing score pre- and post-surgery as secondary outcomes. Results of a clinical study showed that the healing is significantly superior in the insulin group than in the control group
